To use it as a plugin in Lightroom, you mask the mask the object as normal, then select another picture to use as the background while inside Remask (there are some buttons on the bottom left) then you can resize and shift the background to fit.

The ability to load and mask against a background image is nice. I don't see any changes or improvement in the core function. The adjustments are the same and behave the same (hardness, shift, strength, recovery, desat). I don't see any difference in the refine brushes.
